✅ ID & Document Requirements

📌 For OFWs (Overseas Filipino Workers)

1. One (1) valid Philippine government-issued ID

   • Passport (preferred)

   • UMID, SSS, PhilHealth, PRC, Postal, or Driver’s License

2. Work-related document from country of employment

   • Valid Work Visa / Permit

   • Residence ID / Alien ID Card

   • OFW e-Card / OWWA ID

   • Seaman’s Book (for seafarers)

3. Proof of address (any of the following)

   • Utility bill (electric, water, internet)

   • Bank statement

   • Employer-issued residence certificate
     
     

📌 For Filipinos Who Acquired Foreign Citizenship

1. One (1) valid foreign government-issued ID

   • Foreign Passport (required if available)

   • Permanent Resident Card (e.g. US Green Card, Canadian PR)

   • National ID / Residence ID of current country

   • Driver’s License issued abroad

2. Proof of Filipino origin (any of the following)

   • Philippine Birth Certificate

   • Old Philippine Passport (if available)

   • Former OFW documents (e.g. OWWA, POEA record)

3. Proof of current address

   • Utility bill, bank statement, or tenancy agreement in country of residence
